Job Description
Job Summary:
This position is responsible for providing direct support and assistance to individuals in accordance with program policies and procedures.
Job Functions:
Provides support to individuals served by assisting in the development and implementation of Support Plans, assisting individuals in achieving their personal goals, providing meaningful interaction and choices for activities and supporting individuals to participate in daily living skills.
Assists individuals with maintaining the aesthetic of the home and follow safety procedures.
Maintains and expands knowledge skills relevant to providing high quality support and services.
Obtains and maintains the required certifications and trainings to assist with the performance of duties.
Maintains open communication and harmonious work relationship with other staff to assure quality and continuity of support to individuals served.
Reports and documents in a concise, understandable and legible manner routine and unusual events including completing daily logs and accident/incident reports.
Assists with information gathering for personal outcome measures and other assessments.
Ensures that individuals are treated with respect and dignity.
Ensures that individuals exercise their rights and that rights are not restricted without due process.
Assists with meal preparations and laundry.
Assists with accompanying individuals on medical appointments.
Performs any other duties assigned.
Education and/or Experience:
High School Diploma or equivalent. Must possess good verbal, reading and written communication in English and basic math skills. Experience with individuals with developmental disabilities preferred. Must be 18 years of age or older.
Certificates and Licenses:
As applicable, must possess a valid driver’s license and good driving record.
Additional certifications maybe required by regulations.
Clear background checks as defined by regulations and policies.
Physical Demands:
Ability to lift at least 50 pounds.
Ability to do daily standing, bending, stretching and stooping.
